Publicity Consent & Waiver

1. Consent 

 

I irrevocably authorize and consent to the use by Fulfilled Life Institute ("Company," “we," “us," or “our”), of my name, city and state of residence, biographical and personal background, statements, voice, image, and likeness, in any  format, including photos, video, audio, print, or electronic (together, the “Materials”), and grant to Company the unrestricted, perpetual, and worldwide right and license to use the  Materials as Company determines, in its sole discretion, is appropriate to promote and otherwise  carry out its mission. I agree that Company may make the Materials available on its website, in  its publications, or through any other media, including social networking platforms. I understand  and acknowledge that my participation with Company is voluntary, I am not entitled to inspect or approve the use of the Materials, receive notice of their use or publication, or receive any payment.

2. Company’s Rights in the Materials 

 

I grant to Company all copyrights and other rights I may have in the Materials. I acknowledge  and agree that Company owns the physical Materials, as well as the content, any ideas or  information they may generate, and the right to copy, edit, change, transfer, create derivative  works of, or otherwise use the Materials as Company sees fit. I confirm that I have all the rights  and licenses necessary to make these grants. I understand that Company is not obligated to  exercise any of the rights granted under this document.

3. Waiver of Legal Claims 

 

I hereby hold harmless and release and forever discharge Company from all claims, including any claims relating to copyright, rights of publicity or privacy, false light, or moral rights, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.
